Before your first visit
When you schedule your appointment, we will send you our new-patient forms and consent documents so that you can review and complete them at your own pace. If you prefer, you may also share your information through our online registration form.

Your first visit takes place at 315 Madison Avenue, New York, NY 10017, in Midtown Manhattan near Grand Central Terminal, or by secure video for patients located in a state where Dr. Surdhar is licensed.
